The front car of 165124 looks in a pretty bad way as it sits forlornly against an electrification mast that it hit at about 25mph following it overrunning a signal on the entry to Paddington station. Luckily, the train was empty running in from Old Oak Common and nobody, including the driver, were injured. The RAIB report made a number of recommendations relating to driver eating arrangements and to route knowledge. The 165 was repaired at Derby and back in service in its new GWR green livery.
